An incident-centered system that preserves evidence, recommendations, human confirmation and organizational learning across time.

Fast communication is good at conversation and weak at durable operational continuity.

Incidents discussed in messaging channels can lose ownership, follow-up, evidence and historical context as the conversation moves on.

That makes repeated problems harder to recognize and forces people to reconstruct organizational memory from fragmented messages.

An incident becomes the durable unit of operational memory.

The system preserves source evidence, recommendations, human confirmation and incident state as independently understandable assets rather than allowing them to disappear inside a conversation stream.

AI can assist with interpretation and recommendations, while consequential confirmation remains explicitly attributable to people.
